>Conclusion

However, conventional autopsies have some important limitations. They involve surgical cutting of the body, may expose medical personnel to infectious hazards, and may conflict with the religious or cultural beliefs of the deceased person’s family.

Advances in medical imaging and digital technology now provide an important alternative. Virtual autopsy, or Virtopsy, allows forensic experts to examine a body with little or no surgical cutting. Using detailed medical images and three-dimensional digital models, it can reveal injuries and other causes of death while preserving the physical integrity and dignity of the deceased.

What Is Virtopsy?

Virtopsy (virtual autopsy) is a modern post-mortem examination method that uses medical imaging and digital technology instead of relying mainly on surgical dissection. The technique was developed in the late 1990s at the Institute of Forensic Medicine in Bern, Switzerland.

Main Technologies Used in Virtopsy

Post-Mortem Computed Tomography (PMCT): Produces detailed images and 3D models of bones and internal structures. It is particularly useful for detecting fractures, bullet fragments, stab or projectile tracks, internal bleeding, abnormal gas, air embolism, pneumothorax, and blast injuries.

Post-Mortem Magnetic Resonance Imaging (PMMRI): Provides detailed images of soft tissues. It can be useful in examining brain injuries, neurological conditions, heart damage, organ injuries, muscle trauma, and some cases involving children.

3D Surface Scanning: Uses photography, photogrammetry, or optical scanners to record injuries on the surface of the body, such as bruises, abrasions, cuts, bite marks, and weapon impressions. These images can be combined with internal scans to create a detailed 3D picture of an injury.

Post-Mortem CT Angiography (PMCTA): Uses a contrast agent introduced into the blood vessels to identify damaged blood vessels, hidden sources of bleeding, and certain vascular blockages.

Key Advantages of Virtopsy

Preservation of Dignity

Virtopsy can reduce the need for extensive cutting and dissection of the body. This may be especially important where religious or cultural practices discourage invasive autopsy procedures or require rapid burial.

Preservation of the Body’s Original Structure

A conventional autopsy can change the position of organs, fluids, and other structures during dissection. Virtopsy records many injuries in their original position. This can help preserve information about bullet paths, fractures, internal bleeding, and other traumatic injuries.

Permanent Digital Record

Unlike a traditional autopsy, which is partly destructive and cannot be repeated in exactly the same way, Virtopsy creates digital records that can be stored and reviewed later. These records can be shared with other experts, used for independent review, and revisited in cold cases or future investigations.

Improved Occupational Safety

Virtopsy can reduce the need for direct handling and cutting of the body and may therefore reduce occupational exposure to blood, body fluids, and infectious agents. This is particularly relevant when dealing with bodies carrying serious infectious diseases.

Integration with Artificial Intelligence

Artificial intelligence and machine-learning tools can assist forensic experts by helping to identify and classify fractures, measure organs and fluid volumes, segment anatomical structures, and analyse possible ballistic trajectories. These tools can improve the speed and consistency of forensic examination, although final interpretation should remain under expert supervision.

Evidentiary and Legal Relevance in India

India’s modernised criminal justice framework—anchored by the Bharatiya Nagarik Suraksha Sanhita, 2023 (BNSS) and the Bharatiya Sakshya Adhiniyam, 2023 (BSA)—places unprecedented emphasis on objective, technology-led forensic investigation. Virtopsy aligns closely with this statutory direction across three key dimensions:

Scientific Forensic Mandate: Section 176(3), BNSS

The statutory requirement for forensic expert intervention and mandatory audio-video recording in offences punishable by seven years or more underscores the legislative shift toward objective science. Virtopsy directly reinforces this mandate by generating non-destructive, verifiable, and permanent digital datasets.

Admissibility of Electronic Records: Sections 61 and 63, BSA

High-resolution CT and MRI datasets (DICOM format) constitute electronic records. When secured through cryptographic hash values, verifiable audit trails, and the requisite statutory certificate under Section 63 of the BSA, these digital records satisfy the standards of authenticity and data integrity required for court admission.

Demonstrative Evidentiary Value

Three-dimensional digital reconstructions translate complex medical-pathological jargon into clear, intuitive spatial models for judges, prosecutors, and defence counsel—greatly simplifying the presentation of fracture mechanics, internal haemorrhages, and ballistic trajectories.

However, digital imaging data does not enjoy automatic admissibility. Strict evidentiary compliance—including verified chain of custody, authenticated metadata, anti-tampering protocols, and formal radiologist interpretation—remains mandatory.

In contemporary Indian jurisprudence, Virtopsy findings derive admissibility under the intersecting principles of expert opinion (Section 39, BSA) and electronic records (Sections 61 and 63, BSA). Yet judicial recognition remains nascent; there is no authoritative precedent from the Supreme Court or High Courts establishing virtual autopsy reports as complete statutory substitutes for conventional post-mortem reports under Section 194 of the BNSS. A definitive appellate ruling settling its autonomous evidentiary weight is still awaited.

International Jurisprudence

Although high-court judgements on Virtopsy are still limited, courts around the world widely accept it in practice. Swiss courts regularly rely on CT and MRI scans instead of traditional surgery for straightforward cases. In the United States, criminal courts admit 3D post-mortem scans as expert evidence. Similarly, coroners in the UK and Australia frequently use imaging to respect religious objections to invasive autopsies, while German courts rely on scans as part of a combined approach. Overall, courts accept virtual autopsy evidence when the digital files are strictly protected from tampering and the scans are evaluated by qualified forensic and imaging experts. Traditional autopsies are still preferred when dealing with complicated natural deaths.

Limitations and Challenges

Despite its many advantages, Virtopsy cannot completely replace conventional autopsy in every case.

Microscopic and Chemical Limitations

Imaging cannot replace histopathology, microbiological examination, or toxicological testing. It generally cannot identify microscopic cellular changes or determine the exact concentration of drugs, poisons, or other chemicals in the body.

Diagnostic Limitations

Some conditions remain difficult to diagnose through imaging alone. These may include certain early heart-related changes, some superficial burns, and questions relating to drowning that may require tissue examination, laboratory testing, or other forensic methods.

Cost and Infrastructure

Virtopsy requires expensive imaging equipment, specialised software, trained personnel, secure digital storage, and appropriate forensic facilities. These requirements can limit its widespread use, particularly where forensic infrastructure is limited.

The Hybrid Post-Mortem Model

The most practical approach is not to treat Virtopsy and conventional autopsy as competing methods. Instead, they can be combined in a hybrid post-mortem model.

Initial Digital Scan

A whole-body PMCT scan and, where appropriate, 3D surface scanning are performed at the beginning of the examination.

Targeted Assessment

The imaging is reviewed to determine whether it provides sufficient information to explain the cause and manner of death. For example, severe skeletal trauma or a clearly identifiable gunshot injury may be adequately demonstrated through imaging.

Targeted Sampling

If additional information is required, such as toxicological, genetic, microbiological, or histological evidence, the pathologist can perform targeted tissue sampling or limited surgical examination.

This approach combines the strengths of digital imaging with the laboratory and microscopic capabilities of conventional forensic pathology.

Virtual Autopsy Facilities in India

In India, virtual autopsy is primarily conducted at premier medical research institutes. The Centre for Advanced Research and Excellence in Virtual Autopsy at the All India Institute of Medical Sciences (AIIMS), New Delhi, serves as the nation’s pioneering flagship facility and central nodal training hub. The technology has since expanded to key regional tertiary centres, including the North Eastern Indira Gandhi Regional Institute of Health and Medical Sciences (NEIGRIHMS) in Shillong, AIIMS Rishikesh, and AIIMS Guwahati, with several state government medical colleges and tertiary hospitals progressively integrating dedicated post-mortem CT (PMCT) suites.

Public Awareness of Virtual Autopsy (Virtopsy) in India

Public awareness of Virtual Autopsy (Virtopsy) in India remains low and is mostly limited to medical and legal professionals.

While nearly 97% of forensic experts know about the concept, only about 15% fully understand the procedure, and very few have practical experience using it. Among medical and dental students, awareness is mixed—surveys show that between 59% and 75% have either limited knowledge or still favour traditional surgical autopsies.

For the general public, awareness is minimal. Most people only hear about Virtopsy during media coverage of high-profile cases or announcements from premier institutes like AIIMS. Although new centres are opening in places like Lucknow, Bengaluru, and Kerala, the technology is still far from becoming common public knowledge.

Virtual Autopsy vs Traditional Autopsy

Aspect

Virtual Autopsy (Virtopsy)

Traditional Autopsy

Nature

Non-invasive or minimally invasive

Fully invasive; involves surgical dissection

Method

CT, MRI, 3D surface scanning, and PMCTA

Scalpel-based external and internal examination

Body Integrity

Largely preserved; no major surgical cuts

Permanently altered by dissection

Time Required

Usually about 30–60 minutes, depending on the case

Usually 2–6 hours or more, depending on complexity

Infection Risk

Lower risk due to reduced direct tissue handling

Higher risk due to direct contact with tissues and body fluids

Religious/Cultural Acceptance

Generally, it is more acceptable because it causes minimal physical disruption.

May face resistance where religious or cultural practices discourage dissection

Detection Strengths

Excellent for fractures, foreign bodies, abnormal gas, bullet paths, and major internal bleeding

Better for direct examination of soft tissues, histopathology, and subtle organ changes

Limitations

Limited in toxicology, microscopy, microbiology, and some subtle soft-tissue conditions

Invasive and irreversible; dissection can alter the original position of tissues and fluids.

Records

Permanent digital records, such as DICOM imaging data, can be stored and reviewed later.

Findings are documented through notes, photographs, diagrams, and tissue samples; the original dissected state cannot be recreated.

Cost & Infrastructure

High initial cost for scanners, software, storage, and trained personnel

Lower equipment requirements but more dependent on specialised personnel and facilities

Court Presentation

3D images and reconstructions can clearly demonstrate injuries and anatomical relationships.

Relies mainly on photographs, autopsy reports, diagrams, and expert testimony

Best Use

Trauma, accidents, gunshot cases, fractures, foreign bodies, and cases where minimal intervention is preferred

Complex natural deaths, suspected poisoning, histopathological examination, and cases requiring detailed tissue analysis

Current Status in India

Mainly supplementary or part of a hybrid approach; use is developing

Remains the primary and established method of forensic post-mortem examination

Important: The exact time, diagnostic capability, cost, and legal value of either method can vary considerably from case to case. It combines medical imaging, three-dimensional reconstruction, digital technology, and forensic pathology to examine the body with minimal physical disruption.

Virtopsy can help preserve the dignity of the deceased, improve the safety of forensic personnel, preserve the original position of injuries, and create a permanent digital record for later examination and expert review.

However, it is not a complete substitute for conventional autopsy. Histopathology, toxicology, microbiology, and other laboratory examinations may still be necessary in many cases.

The future of forensic medicine therefore lies not in replacing the scalpel completely, but in using digital imaging and targeted physical examination together. A hybrid approach can provide a more efficient, scientifically sound, and legally defensible method of investigating death while preserving both the integrity of the evidence and the dignity of the deceased.

Digital Scalpels and Permanent Truths: How Virtual Autopsy is Changing Forensic Investigation

Is Post-Mortem Examination Always Necessary in Unnatural Death Cases?

Proving a Post-Mortem Report in Court: Turning Medical Findings into Legal Evidence

The Limits of Post-Mortem Reports: What Forensic Evidence Cannot Establish

The Silent Witness: Autopsy, Forensic Science, and the Search for Truth After Death

Introduction For centuries, forensic pathologists have relied on a basic principle: finding the cause of death often requires a traditional autopsy.
